This new working group has been formed as part of our response to Integrated Chronic Disease Management. It has incorporated the Wangaratta Harp Alliance. Four 'Local ICDM Alliances' will be formed and supported to plan local responses to ICDM. There will be one in each of our local government areas; Alpine, Benalla, Mansfield and Wangaratta. These groups will report back to, and be part of the wider CHPCP ICDM Alliance.
